What is a School-Based Health Center (SBHC)?

A school-based health center (SBHC) provides primary care health services and programs within a school setting. The goal of Merlo Station SBHC is to provide care for any and all Merlo Station HS students (including CEYP children) in need of medical and/or mental health services, regardless of insurance status. The NHC-Merlo Station SBHC is run by Neighborhood Health Center (NHC) within the Beaverton District's guidelines, with support from the Washington County Health Department. The NHC-Merlo Station SBHC is separate from the Merlo Station HS School Nurse.

NHC will work with existing providers as needed to maintain continuity of care. All meetings are confidential according to HIPAA guidelines, unless there is a concern for imminent danger to self or others, in which case the student will be made aware of the need to breach confidentiality.

Services Offered at the SBHC

  • COVID Testing
  • Diagnosis and treatment of minor illness, minor injuries, colds, ear infections, rashes, asthma, sinus infections, and may include: limited labs, prescriptions, and handouts/information.
  • Wellness/health physical exams or check-ups
  • Sports physicals
  • immunizations
  • Blood pressure or vision screenings
  • Referrals to specialty providers, i.e., dental, vision, dermatology, orthopedic, etc.
  • Mental health evaluations, counseling, and referrals.
  • Health counseling on topics such as nutrition, tobacco use/cessation, weight management, safety, wellness, substance use/abuse, and health risk assessment/identification/reduction.
  • Addressing safe and responsible sexuality, menstrual disorders, pregnancy testing.

Parental Consent

The SBHC staff will obtain the written or verbal consent of a parent or legal guardian for general medical treatment if a patient is under 15 years of age. A patient may seek family planning and sexually transmitted disease services without parent/legal guardian con​sent, at any age. SBHC-related mental health services (outpatient services) may be provided to a person 14 years or older without parent or legal guardian consent.​


NHC Merlo Station SBHC Satisfaction Survey - Parental Notification

Students between the ages of 12 and 19 who visit the NHC Merlo Station SBHC may be asked to complete an anonymous survey after their visit. The survey asks about the student’s satisfaction and experience at the NHC Merlo Station SBHC, as well as some general questions about their physical and mental health status. Students can refuse to take the survey and this will not affect their ability to get care at the SBHC.
